What Is a Sports Physical?

A sports physical (also called a pre-participation physical exam) is designed to ensure your child is healthy and safe to participate in athletics. During the visit, we will:

  • Review your child’s medical history, including any past injuries, illnesses, or conditions
  • Discuss medications, allergies, and family health history
  • Perform a thorough physical exam, including heart, lungs, vision, joints and overall health.
  • Screen for any conditions that may affect sports participation or require further evaluation
  • Address any concerns about performance, nutrition, hydration, or injury prevention

Conveniently Included in Annual Well Visits

The best part? Sports physicals can be completed during your child’s annual well child visit – no need for a separate appointment in most cases. This allows us to take a comprehensive look at your child’s health while also completing required sports paperwork.

Need a Sports Form Completed?

If your child is up to date on their annual well visit, we can complete sports physical forms – often without needing another full exam. Sports physical forms are free if requested via the portal and $10 if a separate form needs to be completed – just send us the form or bring it by the office. No need to visit urgent care or pay for an extra appointment – we’ve got you covered.

We recommend scheduling your child’s annual well visit ahead of sports season to avoid last-minute stress. Summer is a particularly busy time, so planning ahead helps ensure your child is ready to participate without delays.
